About the App
CCCPH — Backflow Hazard Assessment is an iPhone app built for certified Cross-Connection Control Specialists documenting hazard assessments of public water system service connections. It follows the California CCCPH hazard assessment forms, HA-1 through HA-8, and is field-tested on live public water systems in Southern California.

Subscription & Billing
What does the subscription include?
CCCPH Pro unlocks the complete HA-1 through HA-8 assessment forms, the Connection Register, site photo capture, GPS location tagging, branded PDF report export, deficiency notification letters, and CSV data export.
What are the plans and prices?

How do I cancel or manage my subscription?
All subscriptions are managed through Apple, not through us. On your iPhone: Settings → your name → Subscriptions → CCCPH. You can also open apps.apple.com/account/subscriptions in a browser.
I already subscribed but the app is asking me to subscribe again.
Tap Restore Purchases on the subscription screen. Make sure you're signed in with the same Apple Account you used to subscribe. If it still doesn't unlock, email us and we'll help.
Can I get a refund?
Refunds for App Store purchases are handled by Apple. Request one at reportaproblem.apple.com.
Using the App
Do I need an account?
No. There is no registration or sign-in. Everything you enter stays on your device.
My HA-1 system information is blank on a form. Why?
HA-1 pulls your specialist and company details from Settings at the moment an assessment is created. If Settings was empty when the assessment was started, HA-1 will be blank on that record. Complete Settings first, then create the assessment. See the Field User Manual, Section 2.
How do I get my company branding onto reports?
Open Settings in the app and fill in your company name, logo, specialist name, credential number, credential expiration, D-number, and contact details. These appear in the header and footer of every exported PDF.
How do I load a utility's connection list?
Save the connection list as a CSV file with an address column and an account or meter column. In the app, select the water system, tap Register, then Import CSV. Starting an assessment from a register row pre-fills the address, account number, service type, and line size. Full instructions are in the Field User Manual, Section 4.
The file picker won't let me select my CSV.
Save the file to Files → On My iPhone rather than iCloud Drive. If it still won't open, use the Paste CSV button instead — copy the contents of the CSV and paste them in. The result is identical.
Why aren't GPS coordinates showing on my report?
The app needs location permission to record coordinates. Check Settings → Privacy & Security → Location Services → CCCPH and set it to While Using the App. Coordinates are captured when an assessment is created, so assessments created before permission was granted won't have them.
Does the app work without a signal?
Yes. Assessments, photos, and PDF generation all work fully offline. A connection is only needed for the subscription itself.
Where is my data stored? Is it backed up?
All assessment data, photos, and settings are stored locally on your device and are never transmitted to us or to any third party. Because nothing is stored in the cloud, deleting the app deletes your data. Use Settings → Export all data (JSON) at the end of each field day and keep the file off the device.
What framework do the forms follow?
The HA-1 through HA-8 forms follow the structure of the California Cross-Connection Control Policy Handbook, issued by the State Water Resources Control Board, Division of Drinking Water, and align with generally accepted practice as described in AWWA M14. The app is a documentation tool — it does not issue regulatory determinations or approvals.
